Interaction of social and linguistic constraints on two vowel changes in northern England

This paper focuses on the way that local social indexicality interacts with principles of vowel change. A combination of real and apparent time data from the northern English dialect of York indicate fronting of tense back vowels in the goat and goose lexical sets, and diphthongization of traditionally monophthongal mid-vowels in the face and goat lexical sets. The latter process of change, a northward diffusion of more prestigious southern forms, has been noted for some other northern English dialects, but has not been described acoustically in published work. We show that these two vowel changes have different social meanings in the community. As is the case in previous studies, goat and goose fronting is not strongly associated with different speaker groups in the community. Monophthongal realizations of face and goat, on the other hand, are strongly associated with the speech of the local community, especially working-class speech. The results align with predictions of Labov's (1994) principle III of vowel change in that they show that goose fronting precedes goat fronting. However, we argue that a full understanding of the trajectories of change requires attention to social indexical properties of these variants as well. In particular, the scarcity of fronted variants of monophthongal goat is explained as a consequence of local indexing of such forms.

There is evidence to suggest that GPs experience significant difficulties associated with lack of support from, and communication with, hospital and other specialists in palliative care. The establishment of cancer and palliative care facilitator schemes by Macmillan Cancer Relief reflects these current concerns very clearly. This paper presents some of the findings from the first phase of an evaluation of one such GP facilitator scheme in the Welsh county of Powys. It examines the perceptions and expectations of GP facilitator post-holders during the first year of the facilitator scheme, based upon data gathered from qualitive interviews conducted on three separate occasions. The picture that emerges from the first year of this facilitator project is broadly a positive one. Facilitators have tailored their roles to fit in with and augment the practices with which they deal. Attitudes vary, but their goals of facilitation and education appear similar. Themain challenges facing post-holders is that of being aware of the possible existence of professional rivalries whilst developing their role in ways which do not encroach upon the territories of long-established colleagues.

Let M be a loopless matroid with rank r and c components. Let P(M, t) be the characteristic polynomial of M. We shall show that (−1)rP(M, t)[ges ](1−t)r for t∈(−∞, 1), that the multiplicity of the zeros of P(M, t) at t=1 is equal to c, and that (−1)r+cP(M, t)[ges ](t−1)r for t∈(1, 32/27]. Using a result of C. Thomassen we deduce that the maximal zero-free intervals for characteristic polynomials of loopless matroids are precisely (−∞, 1) and (1, 32/27].

Background
There is a paucity of research into the relationship between insight and variables including cognitive function, educational background and symptomatology in schizophrenia.
MethodSixty-four patients with DSM–III–R diagnosis schizophrenia were assessed with the Schedule for Assessment of Insight, Mini Mental State cognitive test, a new measure of knowledge about treatment (the Understanding of Medication Questionnaire), and educational background and compliance assessments.
ResultsInsight scores correlated significantly with a range of variables. In a multiple regression analysis only knowledge about treatment and number of years in education explained a significant proportion of insight.
ConclusionEducational background and the intellectual ability required to learn complex concepts, such as models of mental illness, appear to be more important than previously considered. Research is limited by the lack of a generally accepted definition of insight.

Background
Various problems are associated with schizophrenia which may theoretically lead to impaired educability about treatment.
MethodThe Understanding of Medication Questionnaire, designed to measure knowledge about treatment in schizophrenia, is described and presented. An educational programme based on a specially designed information booklet was developed. Sixty-four patients with DSM–III–R diagnosis schizophrenia were randomly allocated to groups receiving none (control), one session or three sessions of education.
ResultsPre-intervention low levels of knowledge about illness and treatment increased significantly immediately after a standard education session. Three education sessions led to significantly greater knowledge gain than one session. There was no significant change in the control group. Only the PANSS negative syndrome score independently and consistently explained a significant proportion of the education effect. The influence on educability of attitudes to education, impaired insight, cognitive impairment and other variables were considered. Three sessions of education led to significantly increased insight, but no change in compliance.
ConclusionsTechniques appropriate for educating schizophrenic patients were discussed, and the value of involving patients in education emphasised. A series of patient education sessions is needed to consolidate learning, rather than a single informing process. The strong association between impaired learning and more severe negative schizophrenic syndrome emphasises the need for responsible prescribing of antipsychotic treatment in schizophrenia.

Appetite, weight, and preference for carbohydrate-containing food were followed in patients treated in double-blind trials of seven antidepressant regimes. Trimipramine and isocarboxazid combined had a significant effect in increasing appetite, carbohydrate preference, and weight; in patients who had lost appetite at presentation and were on this treatment, improvement in Hamilton score was linked to increase in weight–a finding not true of the other treatments.

Changes in appetite and weight were recorded in mild to moderately depressed out-patients during the course of clinical trials of various antidepressant drugs. After six weeks, differences due to treatment regimes were minimal. Patients who on becoming depressed experienced severe changes in appetite and weight in either direction, tended to return to normal. The significant associations of appetite and weight change during treatment were with weight, appetite, and carbohydrate preference at presentation, and also with the ‘appetite history’ and ‘weight history’. The associations with the degree of depression were relatively minor.
